Okay Doc, this appears at the least to be an OSHA recordable incident, but if death occurred it must be reported. I while start the root cause incident investigation. I will need to know who witnessed the incident. The date, time, location and a multitude of other things such as what was on your mind just before the incident, was there any impairments (hehehe) were they tools involved that may have contributed and so on.
