Dont know what is going on with the server resets. We reset it manually and it seems to be working now. As far as the missions, the locations are supposed to be selected at random, but they have to meet certain requirements and i guess there are only so many spots that qualify. However, I changed the parameters on mission sites so they should spawn in more diverse locations now. Some spots may be a bit extreme, so be prepared.
