Okay Doc, this appears at the least to be an OSHA recordable incident, but if death occurred it must be reported. I while start the root cause incident investigation. I will need to know who witnessed the incident. The date, time, location and a multitude of other things such as what was on your mind just before the incident, was there any impairments (hehehe) were they tools involved that may have contributed and so on. So once I log in we'll need to spend a few hours together to get this done, okay? But for 10 BC's I can make it all disappear.... we'll talk. (No one else read the last part please.)
